Add a "Create Custom Exercise" form accessible from the exercise library page. Required fields: name, exercise_type, measurement_type. Optional fields: muscle_group_id, equipment. On submit, insert into exercises with is_custom=true and created_by set to the logged-in user. New custom exercise should appear in the search list immediately, tagged with the "Custom" badge.
